Optimizing Epidural Fentanyl Loading Dose for Early Labor Pain

Continuous epidural infusion of local anesthetics and opioids is widely used in clinical practice to relieve labor pain.1,2 The combination of local anesthetics with fentanyl allows for lower doses of both drugs, while maintaining adequate analgesia.3,4 Chestnut et al reported that continuous epidural infusion of 0.0625% bupivacaine plus 0.0002% fentanyl (2 μg/mL) at a rate of 12.5 mL/hr produced analgesia similar to that provided by epidural infusion of 0.125% bupivacaine alone with less intense motor block.5 Labor pain has two components, visceral and somatic pain. During early-stage labor, the visceral pain associated with uterine contraction and cervical dilation usually requires analgesic coverage to contain T10 to L1 dermatomes. Although previous studies reported that the combination of a low-concentration local anesthetic with different doses of fentanyl provides good epidural analgesia,6,7 no study has yet evaluated the analgesic effect of different epidural fentanyl doses on the dermatomic level during early labor. The aim of this study was to examine whether increasing the loading dose of epidural fentanyl (4 μg/mL) could yield analgesia that covers more dermatomes and provide better analgesia for visceral pain during early labor.
